Ichigo’s second level hollow form was not all positive because he was having difficulty controlling himself. It’s an an effect of his hollow powers taking over him including his subconscious mind. While Ichigo is all over Ulquiorra, Ishida tried to stop him because he saw with his two eyes the pity situation of the first . It’s a split second scenario but long enough time for Ulquiorra regenerate himself and gather enough strength to counter-attack of Ichigo while he has divided attention. And in a split second, we’ve seen Ichigo got hit with Ulquiorra’s power and the former’s horn is destroyed. Ichigo’s horn was totally broke and fall to the ground unconscious and seemingly dying .

Orihime without hesitation, thinking that Ichigo died, came to the rescue crying. This time, Ulquiorra and Orihime believed that it”s the end of our hero Ichigo. But then again both of them were mistaken because suddenly a white mist encircled Ichigo’s body followed by a white smoke that penetrated Ichigo’s body. The white smoke cured and healed all the wounds of Ichigo including the hole in his chest. It’s an amazing scenario seeing Ichigo gaining total consciousness with that white mist from nowhere. Ichigo as on top of his game and form again. He can easily kill Ulquiorra without much hurdle this time around.
After all the dust has gone, we must be seeing Ichigo the happiest person because the feat he just achieved by defeating Ulquiorra is historic in nature. But on the contrary, we see him sad because he in his mind, he was the one responsible and accountable for stabbing Ishida to death and also the one who cut off Ulquiorra’s arms and legs.
Ichigo thought on to level the playing field, and so, he asked Ulquiorra to cut off his arms and legs too to the delight of the former. But when he was about to do what he was asked to do cutting Ichigo’s arms and legs, his wings began to falter and evaporate like water. Ulquiorra then asked Ichigo to finish him off but the latter declined it. Why? Ichigo will not happy enough to win the fight by default. He want to play fair and square.
